How to Test for Leaky Gut (The Gold Standard Test)

Leaky gut, clinically known as intestinal permeability, is a condition that has gained significant attention in recent years due to its potential impact on overall health and well-being. WHAT IS LEAKY GUT?

THE GOLD STANDARD TEST: LACTULOSE MANNITOL TEST

Among the various methods available, the Lactulose Mannitol Test is widely considered the gold standard for assessing intestinal permeability. Here’s how it works:

    • Patients consume a solution containing two types of sugars: lactulose and mannitol.

    • Lactulose is a larger molecule that normally should not pass through the intestinal barrier.

    • Mannitol is a smaller molecule that is easily absorbed under normal conditions.

  2. Collection of Urine Samples:

    • Over a period of several hours (usually 6 hours), urine samples are collected.

    • The ratio of lactulose to mannitol in the urine is then measured.

  3. Interpretation:

    • A higher ratio of lactulose to mannitol suggests increased intestinal permeability.

    • This indicates that larger molecules (like lactulose) are passing through the intestinal lining, which is a sign of leaky gut.

 

ADVANTAGES OF THE LACTULOSE MANNITOL TEST

  • Accuracy: It provides a quantitative assessment of intestinal permeability, offering valuable insights into the severity of the condition.

  • Non-invasive: The test involves drinking a solution and collecting urine samples, making it relatively simple and safe for patients.

  • Diagnostic Utility: It helps healthcare providers differentiate between leaky gut syndrome and other gastrointestinal conditions with similar symptoms.

  • Cost: In Australia this test costs $160, which makes it an achievable price-point for many.

 

WHY TESTING IS IMPORTANT 

Can’t we just assume we have leaky gut if we have symptoms? 

While the desire to label ourselves with leaky gut and immediately start drinking bone broth is strong - this is not the answer. 

Science tells us that a leaky gut takes 3-6 months to heal, AND the clinically researched dose of nutrients that are effective is quite high.

For example 15-30g/day of glutamine is what has been used in scientific papers. This is quite a lot! Especially if taken for 3-6 months. (** please note glutamine is not recommended if you have epilepsy).

Other nutrients such as zinc carnosine, and Saccharomyces boulardii CNCM I-745 have also shown to be effective at the right dose.
